Waiver & Release of Liability

I, [Participant Name], acknowledge and understand that participation in any and all programs, activities, classes, and events offered by IronForge Fitness (hereinafter referred to as "the Facility" or "IronForge") involves inherent risks of physical injury, including but not limited to:

• Muscle strains, sprains, ligament and tendon injuries, and bone fractures
• Joint dislocations, cartilage damage, and spinal injuries
• Cardiovascular events, including heart attack and stroke
• Concussions, head injuries, and traumatic brain injuries
• Burns, cuts, bruises, and abrasions from equipment or surfaces
• Injuries resulting from contact with other participants
• Injuries from the use of free weights, machines, ropes, rings, and other apparatus
• Heat-related illnesses, dehydration, and exhaustion
• Injury from falls, collisions, or impact during group fitness classes, boxing, MMA, CrossFit, or similar activities
• Aggravation of pre-existing medical conditions or injuries
• Permanent disability, paralysis, or death

I understand that these risks are inherent to physical fitness and sports activities and cannot be entirely eliminated, even with proper instruction, supervision, and maintenance of equipment.

I voluntarily assume all known and unknown risks associated with my participation, whether those risks arise from the negligence of IronForge Fitness, its employees, agents, contractors, or other participants.

In consideration of being allowed to participate in activities at IronForge Fitness, I, for myself, my heirs, executors, administrators, and assigns, hereby fully and forever release, acquit, and covenant not to sue IronForge Fitness, its owners, members, officers, directors, employees, agents, contractors, affiliates, successors, assigns, and other participants (collectively, the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me (including any property in the custody of the Released Parties), while participating in activities at or in connection with IronForge Fitness.

This release extends to all claims regardless of cause, including claims based on the negligence of the Released Parties, except to the extent such exclusion is prohibited by applicable law.

I understand that this release of liability shall be interpreted as broadly as permitted by law and shall survive any injury, disability, or death that I may sustain.

I represent and warrant that:

• I am in good physical condition and am medically fit to participate in physical fitness and sports activities
• I have no known physical or mental conditions, illnesses, disabilities, or medical problems that would make participation dangerous or inadvisable
• I have disclosed all relevant medical information to my physician and have received clearance to engage in vigorous physical activity
• I am not currently under the influence of alcohol, recreational drugs, or any prescription medication that may impair my ability to participate safely
• I understand the importance of proper hydration, nutrition, and rest, and agree to participate in a safe and responsible manner
• I will promptly inform IronForge Fitness staff of any injury, illness, or adverse reaction that occurs during my participation

I understand that IronForge Fitness relies on the accuracy of my representations. If any representation is false or misleading, this waiver may be void, and I may be held liable for any resulting damages.

I agree that IronForge Fitness reserves the right to refuse participation or terminate my participation at any time if, in the judgment of its staff, my continued participation poses a risk to myself or others.

In the event of an accident, injury, or medical emergency during my participation in activities at IronForge Fitness, I hereby authorize IronForge Fitness and its employees to arrange for and consent to emergency medical treatment, including hospitalization, surgery, anesthesia, and any other necessary medical services.

I agree to bear all costs associated with such emergency medical treatment. Nothing in this authorization shall create any obligation on the part of IronForge Fitness to pay for medical treatment.

In the event of an emergency where I am unable to communicate, IronForge Fitness may contact my designated emergency contact listed in the registration form below.

I understand and acknowledge that my image, likeness, voice, and performance may be captured through photographs, video recordings, audio recordings, and other media during my participation in IronForge Fitness programs and events.

I hereby grant IronForge Fitness the irrevocable, worldwide, royalty-free, and perpetual license to use, reproduce, distribute, modify, adapt, publicly display, and publicly perform such media in any format or medium now known or hereafter developed, for purposes including but not limited to:

• Marketing, advertising, and promotional materials
• Social media platforms and digital content
• Websites, brochures, and printed materials
• Internal training and educational purposes
• Public relations and media coverage

I agree that I shall not receive any compensation or royalties for such use. I acknowledge that IronForge Fitness may use my name, testimonial, and results data in marketing materials.

I agree to abide by all IronForge Fitness rules, policies, and codes of conduct, including but not limited to:

• Using all equipment properly, safely, and for its intended purpose
• Following all instructions from coaches, trainers, and staff
• Re-racking weights and returning equipment after use
• Wearing appropriate footwear and attire during all activities
• Maintaining appropriate hygiene, including wiping down equipment after use
• Not bringing glass containers, outside food, or prohibited items into the facility
• Treating all participants, staff, and property with respect
• Not engaging in horseplay, dangerous behavior, or unauthorized activities
• Adhering to all posted signage and area restrictions

I understand that failure to comply may result in immediate removal from the facility, suspension, or permanent termination of my membership, at the sole discretion of IronForge Fitness.

If the participant is under the age of 18, the undersigned parent or legal guardian, on behalf of myself and my minor child(ren), hereby acknowledges the risks described above and grants permission for my child(ren) to participate in activities at IronForge Fitness.

I further agree to all terms, conditions, releases, and authorizations set forth in this waiver on behalf of my minor child(ren), and assume full responsibility for their conduct and well-being.

I certify that I am the legal parent or guardian of the minor participant and have the legal authority to bind the minor to these terms.

This Waiver and Release shall be governed by and construed in accordance with the laws of the State of [Your State], without regard to its conflict of law provisions.

I agree that any legal action or proceeding arising under or relating to this agreement shall be brought exclusively in the state or federal courts located in [County], [State], and I hereby consent to the personal jurisdiction and venue of such courts.

If any provision of this Waiver and Release is held to be invalid, illegal, or unenforceable, the validity, legality, and enforceability of the remaining provisions shall not in any way be affected or impaired thereby. The invalid provision shall be modified to the minimum extent necessary to make it valid and enforceable.

I acknowledge that I have read this Waiver and Release in its entirety, understand its terms, and sign it voluntarily and without coercion. I understand that this document has significant legal effect and may limit my rights to sue in the event of injury.
